Researchers in Amsterdam showed 120 people photos of the same volunteer wearing seven different clear aligner systems. Attractiveness scores for six of the systems fell between 63 and 73 out of 100, while one system scored just 34.78 (Livas and colleagues, Dental Press Journal of Orthodontics, 2023).

Same smile, same photo, very different reactions. "Invisible" is a goal, not a guarantee, and what people notice depends on far more than the plastic itself.

Quick answer: will people notice I'm wearing aligners?

Most people will notice very little, but not nothing. Clear aligners are transparent trays, so they are far less visible than metal braces, though small tooth-coloured attachments and a brief change in speech can give them away. In a 2019 study of 199 adults, 86.9% would accept clear aligners for themselves, and standard metal brackets scored significantly lower for attractiveness. A 2024 review found that speech changes usually settle within 7 to 14 days. As of October 2026, the best way to stay discreet is careful planning, a digital preview, and regular progress checks.

What are invisible aligners, and what makes them visible?

Invisible aligners are custom-made, transparent plastic trays worn in a numbered series, each one moving the teeth a little further. They belong to clear aligner treatment, which sits within orthodontics, the branch of dentistry that corrects crowding, gaps and bite problems.

Three things can make them noticeable: the tray itself, small bonded bumps called attachments, and any change in how you speak. Dental braces, by contrast, put brackets and wires on the front of the teeth for everyone to see.

Do aligners look better than metal braces to other people?

Yes, in the studies available. In a 2019 survey of 199 Saudi adults in Patient Preference and Adherence, clear aligners and lingual brackets shared the highest median attractiveness score (8), and standard metal brackets scored significantly lower (P<0.0001) (Alansari and colleagues, 2019).

The same study found that 86.9% of adults would accept clear aligners for themselves. The authors noted that the least attractive appliances were the most visible ones.

Lingual braces, which sit behind the teeth, are another hidden option, but they are a different treatment with their own trade-offs (see lingual braces).

Are all aligner systems equally discreet?

No. In the Amsterdam survey by Livas and colleagues (2023), perceived aligner visibility, acceptability and attractiveness differed significantly between systems. Five of the seven systems were rated acceptable by more than 83% of participants, yet one system scored far lower than the rest on attractiveness.

The lesson is practical: the way a tray is trimmed, how closely it fits and how clear it stays all change what an onlooker sees. Ask to see the actual system on a model of your own teeth, not a brochure photo.

Will people hear a difference when I talk?

For a short time, possibly. A 2024 summary in Evidence-Based Dentistry of a systematic review of 7 studies and 332 patients found that speech difficulties with clear aligners were similar to those with fixed appliances, with the /s/ sound the most affected (Al-Dboush and colleagues, 2024).

Most patients recovered within 7 to 14 days, though some took 30 to 60 days, and lingual fixed appliances caused more difficulty than aligners. The authors rated the evidence as low quality, so treat the timeline as a guide, not a promise.

Can people see the attachments on my teeth?

They can, if they look closely. Attachments are small tooth-coloured bumps bonded to certain teeth so the tray can grip and push them, and they are removed at the end of treatment. A 2022 systematic review in Materials concluded that attachments mostly increase the effectiveness of clear aligner treatment, improving rotation, root torque and mesio-distal movement (Nucera and colleagues, 2022).

That review covered five studies of about 203 patients and noted some contradictory results. So attachments are a trade: slightly more visible teeth in exchange for more predictable movement in many cases.

Does comfort affect how noticeable aligners feel?

It affects how confident you feel, which shapes how noticeable they seem. In a 2024 study of 267 laypeople in the Journal of Dental Research, Dental Clinics, Dental Prospects, 93.6% said they would feel comfortable with clear aligners alone and 97.4% said they would be satisfied (Castilhos and colleagues, 2024).

These were expectations based on photographs, not patient experience, so they show attitude rather than outcome. Even so, the pattern matches the other studies: people tend to view plain aligners favourably.

How do the things people notice compare?

The table below sets out what an onlooker might notice with each option, using the studies above where they apply.

What people may notice Clear aligners Metal braces
Appearance on the teeth Transparent trays; rated highly for attractiveness (Alansari, 2019) Brackets and wires; rated significantly lower (Alansari, 2019)
Extra bumps on teeth Tooth-coloured attachments on some teeth (Nucera, 2022) Brackets on every tooth
Speech Possible short-term change, mostly settling in 7 to 14 days (Al-Dboush, 2024) Similar difficulty reported for fixed appliances (Al-Dboush, 2024)
Differences between systems Visibility and attractiveness vary by system (Livas, 2023) Varies by bracket type

How should a working professional choose a discreet aligner plan?

Choose the provider who plans for discretion before treatment starts, not the one who only promises it. Five criteria matter more than the brand name on the tray:

  • Verified hygiene standards, such as NABH approval, because aligner care means many visits over many months.
  • Digital scanning and a 3D preview so you can judge the planned result and the attachment positions in advance.
  • An experienced team that explains which movements aligners handle well and which need another approach.
  • Several convenient locations, so progress checks fit around a busy work schedule.
  • Modern, comfort-focused technology that keeps visits short and predictable.

What should you check before you start aligners?

Use this short list in any consultation, whichever clinic you visit:

  • Ask to see a digital preview of your planned tooth movement and where attachments will go.
  • Ask which aligner system will be used and why it suits your bite.
  • Ask what to expect with speech in the first two weeks.
  • Ask how often you will be seen, and how the clinic checks that your teeth are tracking the plan.
  • Ask what hygiene accreditation the clinic holds and how instruments are sterilised.

Frequently asked questions

Do aligners make you lisp?

They can for a short time. A 2024 review of 7 studies found the /s/ sound was most affected, and most patients recovered within 7 to 14 days. Some took 30 to 60 days, so ask your dentist what to expect.

Are aligners less noticeable than metal braces?

In the research reviewed here, yes. Adults rated clear aligners significantly more attractive than standard metal brackets in a 2019 survey, and 86.9% would accept them. Individual systems still differ, so compare options in person.

Can people see attachments on aligner teeth?

Up close, they can. Attachments are tooth-coloured and small, and they are removed when treatment ends. Ask for a digital preview so you know which teeth will have them.

Will my colleagues and clients notice aligners?

Most people are unlikely to notice trays at normal conversation distance, but no study can promise that for every person. Speech changes in the first two weeks are the most likely giveaway, so many patients start during a quieter stretch at work.

Does every aligner system look the same?

No. A 2023 survey found that perceived visibility, acceptability and attractiveness differed significantly between seven systems. Ask to see the specific system your dentist plans to use.
